Exit Interview Questions

Does anyone have a list/template of questions used for exit interviews?  We are trying to help a client that is having retention issues get a better understanding of why people are leaving.  Thanks.

    Hi Patrick, 

    Here is a list I prepared taking the best of what many of my clients have used.  You would obviously pick and choose, but it gives some good ideas for questions.

      Charla Stevens Wow! Thank you. Your list of questions is good. I was going to suggest "What was it that caused you to start looking for another opportunity" but you already had it on your list.
