Trying not to recreate the wheel.
ISO: TA Partner Self Assessment Template (3 point system preferred)
This is my first year with current employer and historically, they've relied solely on Hiring Manager surveys, which I am not a fan of.

In my past experience, I created a scorecard with key metrics for the recruiting team. This was then used for performance reviews. Let me know if you want a copy of it.
