Third party company recommendations - Exit Interviews

What I am hoping to learn from other organizations is whether you have experience with third party organization or software programs to conduct exit interviews when staff leave the organization.  If so, how was the feedback delivered to you and were you satisfied with the information provided? What type of costs were involved? Are you still using their services?
