Issue Reporting Outlet

Does anyone have a incident/issue report policy in place? We are looking to establish something that allows employees to submit an incident or report an issue.  We want to allow for some anonymity so that they are more willing to report something that they  may not otherwise do. If so, do you use a dedicated voicemail ?

  • We use a company called "EthicsPoint" which has both an online reporting website and a toll-free number employees (or anyone else) can call.  They do a great job and can provide training, resources, analytics, etc.

  • We use a service offered through NAVEX Global for what we call our Integrity Hotline.  Employees can report violations of our Code of Conduct, unsafe working conditions, or worse.  They can report something online or via the telephone and it's totally confidential.

  • We used a company called "Lighthouse" for ethic concerns.  Similar to the vendors mentioned by others on this thread.  Concerns could be reported via e-mail or phone and could be anonymous.   All concerns and action taken were tracked. Worked well.
