Some ACA form filing questions . . .

We are a large employer and understand we have to file a 1095-C to IRS.  I’m not sure what goes into Part 11 Employee Offer and Coverage line 15.  We have two plans, a PPO Plan and a Health Savings Plan.  The Health Savings Plan is the more reasonable plan of the two plans, do I use the amount for EMPLOYEE ONLY in those blocks?  On the W-2 what goes in block 12DD?

  • Box 12DD on the W-2 is for the total premium paid for the calendar year for the employee's coverage, this includes both the share that the employer paid and any portion that the employee paid towards the total premium billed.

  • For the 1095C -use the lowest cost plan that is offered and the amount for employee only coverage.
